Who needs an immigration evaluation?

Anyone who wants to become a permanent legal resident of the United States needs to get a green card, a process the government calls adjustment of status (AOS). And if you want a green card, you must get an immigration evaluation (physical examination) by a qualified civil surgeon. Dr. Edokpayi at Rapha Health and Wellness serves as a government-approved civil surgeon.

  • What Is the Purpose of Form I-693?

    Form I-693 reports the results of an immigration medical examination to the U.S. Citizenship and Immigration Services (USCIS). USCIS requires the examination to establish that applicants who are seeking immigration benefits are not inadmissible to the United States on health-related grounds.

  • Why are immigration evaluations required?

    Immigration evaluations are designed to detect contagious diseases and conditions that could pose a threat to public health. You’re screened for health concerns such as:
    ● Infectious diseases (tuberculosis, syphilis, and gonorrhea)
    ● Physical or mental health disorders that can lead to dangerous or harmful
    behaviors
    ● Drug abuse or addiction

    If you have a contagious disease, the team at Rapha Health and Wellness treats the condition. Once the infection heals, you can move forward to the next step in getting a green card.

  • Will I get immunizations during my immigration evaluation?

    You must have all the required immunizations for your age. If you’re up-to-date with shots, bring your immunization records to your appointment at Rapha Health and Wellness. If you haven’t had all the required shots, or don’t have a written record of them, the team at Rapha Health and Wellness can administer the vaccinations needed to apply for your green card.

What should I expect during my immigration evaluation?

Your Rapha Health and Wellness provider reviews your medical history to obtain information about:
● Current illnesses
● Chronic health conditions
● Disabilities
● Drug and alcohol use
● Hospitalizations for physical or mental health conditions

Your provider may run an urinalysis to test for drugs. If you’ve had tuberculosis or any health condition and have records about your treatment, it’s important to bring them to your immunization exam.

After learning about your health, your provider evaluates your mental status and performs a physical exam. If they discover any potential problems, they may order blood work or diagnostic tests.

After finishing your exam, your provider completes the paperwork needed for your green card and puts it in a sealed envelope. You submit the envelope (still sealed) along with your application or take it with you to your green card interview.
